You must complete this form if you can’t submit an assignment or complete an evaluation due to an absence from at least one of your courses. You must do this for every course in which you miss an evaluation.

This form doesn’t apply to the following: practicums, internships, clinical placements, laboratories, field work or any type of experiential learning. Neither does it apply if you already have an academic accommodations plan that includes the right to defer assignments or extend deadlines.

Your first absence declaration from a course doesn’t require any supporting documents.
